We work the evidence methodically and without blame, because the goal is a system that does not repeat the failure, not a person to point at.

How we work

  • Reconstruct the timeline. We assemble what happened, in order, from logs, telemetry, and the people who were there.
  • Ask why, repeatedly. Using 5 Whys and fishbone analysis we move past the trigger to the systemic conditions that allowed it.
  • Separate cause from contributor. We distinguish the root cause from the factors that made it worse or slower to detect.
  • Drive corrective action. Each finding becomes an owned, prioritised action with a way to verify it landed.
